Somer Schreiber Small, FNP-C, APRN has been in the healthcare field for over 20 years and has worked in several areas, including acute care, cardiac telemetry, dialysis, and in post-acute care. Somer Schreiber Small’s, FNP-C, APRN most recent work is in seeing patients at several post-acute care facilities, where she utilizes her experience to help patients maintain a healthy lifestyle and mitigate the effects of disease.
She also has a part time weight loss practice. Somer Schreiber, Small, FNP-C, APRN has a passion for helping people achieve their healthcare goals and is a compassionate and caring provider.

I looked into a number of options and did a lot of research on new weight loss techniques before reaching out to Synchronicity. Somer got back to me right away and has been on top of things from that moment on. She worked with specific requests I had for paperwork (work documentation) and has been available whenever I have a question. Weight loss has started to kick in, and I appreciate that she provides thorough guidance on fluids and diet to minimize any side effects (so far none!)

This program is a miracle. If you have an addiction to sugar that makes it hard to lose weight, the medication will help curb cravings and Somer is super supportive throughout the day via text if you have questions or need help with anything. 5lbs gone in 1 week & I have not drank nor do I crave Coca Cola which is huge for me
I have nothing but positive feedback to share! I honestly wish I found her sooner. Somer is a solid professional, results driven, and believes in her work. Her bed side manner makes me feel good-which is super important and hard to find. Being said, exceptional customer service, flexible, up-to-date, and FAST response turnaround.
