SSNP Corp, doing business as Synchronicity Health (“Synchronicity Health,” “we,” “us,” or “our”), respects your privacy. This Privacy Policy explains what personal information we collect through our website and digital services, how we use it, when we disclose it, and the choices you have.
1. Scope
This Privacy Policy applies to information collected through:
- synchronicityhealth.net and the landing pages we operate, including wl.synchronicityhealth.net and hormones.synchronicityhealth.net;
- online contact, consultation, and appointment-request forms;
- our website chat assistant;
- email and text message communications;
- analytics, advertising, and attribution technologies on our website; and
- other digital services operated by or on behalf of Synchronicity Health.
This Privacy Policy does not apply to information collected offline, through third-party websites we do not control, or in your medical record. Your medical record is governed by our Notice of Privacy Practices.
2. Information You Provide to Us
We collect information you choose to provide, which may include:
- first and last name;
- email address;
- telephone number;
- mailing address;
- information you enter in forms, chat, or messages to us;
- appointment and consultation requests, and the services you are interested in;
- communication preferences and email or SMS consent records; and
- any other information you choose to share with us.
Please do not submit sensitive information (such as medical details, Social Security numbers, or financial account numbers) through general website forms or chat unless the form specifically requests it.
3. Health Information, HIPAA, and California Medical Privacy
Synchronicity Health is a healthcare provider subject to the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and the California Confidentiality of Medical Information Act, California Civil Code Section 56 and following (“CMIA”). Information that qualifies as Protected Health Information (“PHI”) under HIPAA or as medical information under the CMIA is governed by our Notice of Privacy Practices, which explains how we use and disclose it and your rights regarding it. If this Privacy Policy and our Notice of Privacy Practices conflict regarding that information, the Notice of Privacy Practices controls. Where California law gives your medical information more protection than HIPAA, we follow California law.
Health information on our website. Information you provide while requesting or receiving care, such as symptoms, treatments of interest, medications, medical history, lab results, or appointment details, may be PHI. Please do not submit detailed medical information through general contact forms or chat. Use our secure patient portal instead.
Tracking technologies and health information. We do not authorize advertising, analytics, chat, visitor identification, or other technology providers to receive or use PHI or medical information except as permitted by HIPAA and the CMIA and, where required, under a Business Associate Agreement. We configure our website so that information entered in patient intake, scheduling, or portal pages is not sent to advertising platforms. A cookie banner or this Privacy Policy does not by itself authorize any disclosure of PHI or medical information that HIPAA or the CMIA would otherwise prohibit.
Treatment, payment, and operations. We may share health information with other providers, laboratories, pharmacies, and insurers as permitted by HIPAA and the CMIA and described in our Notice of Privacy Practices.
We never sell PHI or medical information.
Medical emergencies. Our website, forms, email, text messages, social media, and chat are not monitored for emergencies. If you are experiencing a medical emergency, call 911.
4. Information Collected Automatically
When you visit our website, certain information is collected automatically, including:
- IP address and approximate location derived from it;
- browser type, device type, and operating system;
- referring website, search terms, and campaign information (such as UTM parameters and ad click identifiers);
- pages viewed, links clicked, time on page, and navigation paths;
- form and button interactions; and
- cookie, device, and advertising identifiers.
5. Cookies, Pixels, and Similar Technologies
We use cookies, pixels, tags, and scripts for the following purposes.
Essential. These support site security, form submission, and basic functionality.
Analytics. These help us understand how visitors find and use our website. We use Google Analytics. You can opt out using the Google Analytics Opt-out Browser Add-on at tools.google.com/dlpage/gaoptout.
Advertising and attribution. These help us measure whether our ads lead to website visits, calls, or form submissions, and may be used to show our ads to people who have visited our website. Providers may include Google, Meta, and Microsoft. These providers may collect information about your activity on our website and across other websites over time. You can limit interest-based advertising at optout.aboutads.info, through your device settings, and through each platform’s ad settings.
Marketing automation. Our forms, calendars, CRM, email, and text messaging run on GoHighLevel and similar service providers acting on our behalf.
You can block or delete cookies through your browser settings. Some site features may not work properly if you do.
6. Website Visitor Identification
We use a visitor identification service, Visitor Connect, provided by Inbound Insight, LLC, to understand who visits our website.
Technologies used. Our website runs a pixel, a small piece of code, that may use cookies or similar technologies to collect information from your browser and device. This includes your IP address, browser and device type, the pages you view, the date and time of your visit, and the website or ad that referred you. We do not use visitor identification services on patient portal, intake, or scheduling pages.
Information collected. The provider may match this activity against information lawfully available to it from third-party and public sources. Matched information may include your name, mailing address, email address, and phone number.
How we use it. We use this information to measure marketing performance and, where legally permitted, to contact you about our services, including by mail or email. Matched information is stored in our CRM and shared with service providers, such as direct mail vendors, only to send these communications.
Your choices. You can block or delete cookies through your browser settings. To stop us from contacting you, or to have this information deleted, contact us at info@synchronicityhealth.net.
7. Website Chat Assistant
Our website offers a chat assistant powered by artificial intelligence. The assistant identifies itself as AI at the start of every conversation. It is not a person, and it is not a nurse practitioner, physician, nurse, or other licensed healthcare professional. Responses are generated automatically and may not always be accurate or complete. Chat conversations, including anything you type, are recorded and stored by us and our service providers, and may be processed by third-party AI providers to generate responses. By using the chat assistant, you consent to this recording and processing. The chat assistant does not provide medical advice and cannot be used for emergencies. Do not use chat to report symptoms or request prescriptions, and please do not share sensitive personal information in chat.
8. How We Use Personal Information
We use personal information to:
- respond to inquiries and requests;
- schedule and manage appointments and consultations;
- provide the services you request;
- send transactional messages such as confirmations, reminders, and updates;
- send marketing communications where you have consented or where permitted by law;

- comply with legal obligations and enforce our rights.
9. Email Communications
If you provide your email address, we may send you messages about your inquiry, appointments, services, and, where permitted, promotions or educational content. Every marketing email includes an unsubscribe link. Unsubscribing from marketing does not stop transactional or service-related emails.
10. Text Messaging (SMS)
If you provide your mobile number and check the consent box on our website forms or in our chat assistant, you agree to receive text messages from Synchronicity Health at the number provided. Messages may include appointment confirmations and reminders, responses to your inquiry, service updates, and, if you separately consent, promotional offers.
- Message frequency varies.
- Message and data rates may apply.
- Reply STOP to opt out at any time. Reply HELP for help.
- Consent to receive text messages is not a condition of purchase.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. Information may be shared with service providers that help us deliver messages, such as our messaging platform and carriers, solely to provide the service.
11. How We Disclose Personal Information
We disclose personal information only as described here:
- Service providers that host our website, run our CRM, forms, email, SMS, chat, call tracking, direct mail, scheduling, analytics, advertising measurement, payment processing, and IT security, and that are bound to use the information only to provide services to us. Service providers that create, receive, maintain, or transmit PHI on our behalf sign a Business Associate Agreement.
- Advertising and analytics platforms through the technologies described in Sections 5 and 6.
- Legal requirements, including to comply with law, subpoena, or court order, to respond to government requests, or to protect the rights, safety, or property of Synchronicity Health or others.
- Business transfers, as part of a merger, acquisition, financing, or sale of assets.
- With your consent or at your direction.
12. Sale and Sharing of Personal Information
We do not sell personal information for money. Some state laws define the use of advertising cookies and pixels as a “sale” or “sharing” of personal information even when no money is exchanged. Where those laws apply to you, you may opt out by contacting us at info@synchronicityhealth.net, and we will honor Global Privacy Control browser signals where required by law.
13. Third-Party Websites
Our website may link to websites, booking tools, portals, payment pages, and social media platforms we do not operate. Their privacy practices are governed by their own policies.
14. Data Security
We use reasonable administrative, technical, and physical safeguards to protect personal information, including access controls, secure hosting, and vendor management. No website or transmission over the internet is completely secure. If a security breach affects your personal information, we will notify you as required by California law, including Section 1798.82 of the California Civil Code, by HIPAA, and by any other applicable law.
15. Data Retention
We keep personal information only as long as reasonably necessary to provide services, maintain business records, meet legal, tax, and regulatory requirements, keep consent records, resolve disputes, and enforce our agreements. Medical records are kept as required by HIPAA and California law.

If you believe a child has submitted information to us, contact us and we will delete it. Care for minors is handled under applicable parental and guardian consent laws.
